Clinical Hospital Center Rijeka in the Primorsko-Goranska region of Croatia has identified two major challenges that affect patient safety and hospital efficiency. To address these, two SMEs have been chosen for each challenge—one as the leading SME from Croatia and another from a different region of the consortium to bring interregional innovation. These partnerships aim to improve hospital operations and patient outcomes through cutting-edge technology and collaborative solutions. Here are the four companies selected to co-create solutions for these challenges in the Primorsko-Goranska region.
Hospital Challenge 1: FallPredict

Falls among elderly and vulnerable patients are a leading cause of injury, prolonged hospital stays, and increased healthcare costs. At CHC Rijeka, fall prevention has become a top priority, as current methods of predicting and preventing falls are inadequate. A more precise tool for assessing patient fall risks is urgently needed to prevent injuries and improve patient safety.

StabilityGuard is a sophisticated system designed to prevent and detect falls, in conjunction with the KBC Rijeka and two IT Companies selected.. By employing state-of-the-art technology to monitor, forecast, and promptly address fall incidents, this system aims to paradigm shift patient safety in healthcare settings. The innovation of StabilityGuard resides in its ability to incorporate novel insights from the domains of healthcare, machine learning, and wearable technology. Through the utilization of real-time data analysis and predictive algorithms,StabilityGuard empowers healthcare providers to proactively avert mishaps, thereby signifying a substantial advancement in the provision of patient care and safety.

Consector Biro will work closely with Sparky to define the project’s scope and objectives, ensuring thorough planning and strategic alignment. They will also focus on establishing key performance indicators (KPIs) to monitor project progress. Additionally, Consector Biro will assist in developing an advanced AI model for accurate fall prediction using data collected from devices. This support includes guidance on selecting algorithms, preparing data samples, and creating secure data handling protocols to protect patient information. Furthermore, they will aid in solution validation by conducting survey research with physicians and seniors using established fall screening tools.

Effective communication between different departments within the hospital is essential for smooth patient transfers and coordinated care. Currently, CHC Rijeka faces challenges in ensuring seamless communication between medical teams during patient transitions, which can lead to delays and errors in treatment. A reliable communication platform is needed to streamline these processes and improve care coordination.

Bit4Health is advanced healthcare communication platform designed to deliver specialized cardiac care, this solution focuses on enhancing remote consultations, collaboration, and patient outcomes. The platform will enable healthcare teams to share real-time patient information, track transfers, and ensure that all care providers have access to the latest data. This will enhance workflow efficiency, reduce treatment delays, and improve patient care outcomes.
