HealthChain is an interregional collaborative project that spans 10 European regions within 8 countries and focuses on developing and implementing 24 innovative digital solutions designed to address critical needs in modern healthcare. HealthChain aims to create sustainable and effective digital solutions that empower patients and transform care delivery by fostering collaboration between hospitals, SMEs, ecosystem supporters, and other stakeholders.
HealthChain offers a model for co-created, demand-driven technological solutions in health care by creating optimal value-chains. It provides a framework in which innovations arise from the real-life needs of the patients and medical professionals.
To address specific healthcare needs across the participating regions, 33 SMEs are developing 24 digital solutions. These solutions draw upon a variety of technologies, encompassing mobile applications, wearable devices, AI-powered platforms, and telemedicine. Some of the solutions being developed include:
- Remote Patient Monitoring: Several solutions focus on enabling the remote monitoring of patients with chronic conditions, such as heart failure (e.g., “Remote Monitoring for Heart Failure Patients”) and chronic kidney disease (e.g., “KONTROL-IT”). These solutions use wearable sensors and other devices to track vital signs and other relevant parameters, allowing for early detection of deterioration and timely interventions.
- AI-Powered Diagnostics: AI is being utilized to improve the accuracy and efficiency of diagnostics. For example, the “AITRIS” solution assists radiologists in detecting lung nodules in CT scans, while the “CARDIOLOGY AI PREDICTIVE MODEL” helps manage complexities in aortic stenosis treatment.
- Telemedicine and Communication: Solutions like “Bit4Health” and “BeterDichtbij Collega” aim to improve communication and collaboration among healthcare professionals and between doctors and patients.
- Patient Empowerment: Several solutions empower patients to actively manage their health. “ESFERA” and “PALDOC” provide tools for self-management of conditions like chronic pain.
- Specific Disease Management: The project also addresses specific diseases and conditions. For instance, “Ophthalmology NLP” and “Ophthalmology Collect” focus on improving care for ocular diseases, while “EMOPALL” supports remote monitoring of palliative care patients.
- Rehabilitation and Prevention: Solutions like “MobileCare+” and “MoveBuddy” focus on improving rehabilitation outcomes and promoting preventative health measures, such as addressing childhood obesity.
